Spleen filters blood and fights infections; its removal is permanent immunocompromise. Requires lifelong vaccinations and sometimes prophylactic antibiotics. Risk of overwhelming post-splenectomy infection (OPSI), potentially fatal.
Automatic 20% rating for splenectomy regardless of reason for removal. Separately rate complications such as systemic infections with encapsulated bacteria.
