Tick-borne illness caused by Borrelia bacteria. Early infection causes erythema migrans rash, fever, fatigue, headache, joint pain. If untreated, disseminates causing neurological (facial palsy, meningitis), cardiac (heart block, myocarditis), and joint (Lyme arthritis) manifestations. Post-treatment Lyme disease syndrome causes persistent symptoms despite treatment.

Incapacitating manifestations. Severe encephalopathy, complete heart block, or other life-threatening complications (temporary rating during active severe disease)

Severe symptoms. Chronic neurological involvement (encephalopathy, peripheral neuropathy), chronic arthritis, or cardiac involvement causing severe functional impairment

Moderately severe symptoms. Recurrent arthritis, peripheral neuropathy, or cognitive dysfunction significantly limiting function

Moderate residual symptoms. Persistent arthritis, mild neuropathy, or chronic fatigue affecting daily activities

Mild residual symptoms. Occasional fatigue or joint pain not significantly limiting function
