With remaining visual field of 5 degrees or less
With visual field defect that includes contracted peripheral field with remaining visual field of 5-10 degrees
With visual field defect that includes contracted peripheral field with remaining visual field of 10-20 degrees or corrected visual acuity 20/200 or less in better eye
With visual field defect that includes contracted peripheral field with remaining visual field greater than 20 degrees or corrected visual acuity 20/50 to 20/100 in better eye
With visual field defect limited to an enlarged blind spot or scotomas in peripheral field, or corrected visual acuity 20/40 or better in each eye
